Release of Information
I authorize the College of Saint Benedict and its volunteers to use my Bennie Mentor Program application information to match me with a Bennie alumna and to share my application information with my match. I am providing this information to help the Alumnae Board find a Bennie match for me. I understand that the College of Saint Benedict may use this information, without name attribution, for statistical or demographic purposes.

Agreement
By completing this form, I understand that I am entering into a mentoring relationship with a Bennie alumna who will be assigned to me by September 2023.

• My commitment is to connect with my mentor/mentee a minimum of once per month during the nine-month duration of the program, Sept. 2023-May 2024, and to conduct myself in a professional and appropriate manner throughout the relationship.
• After looking at my personal and professional schedule, I have made sure that I will have adequate time to devote to the mentor relationship throughout the full nine months.
• I understand that based on the applicant pool, my match may be younger, older or the same age as me. I also understand that my match may not live in the same area as me and it may require we meet virtually. 

I agree to complete a Bennie Mentor Program evaluation after the program ends, and I will do my very best to attend the Bennie Mentor Program Orientation & Kick-Off event in September, in-person or virtually.
